The dominant physical feature is severe, left-sided pain — in the spine, chest, and extremities simultaneously. Spinal pain is sharp and sensitive to touch, particularly in the upper back and neck. Chest pain has an anginal quality — constricting, left-sided, and accompanied by dyspnoea. Numbness and a cold, dead feeling in the left limbs add to the picture. Clarke notes the sharp, lightning-like quality of the neuralgic pains throughout the left side.
